When can I wear eye makeup after a corneal transplant?

Short answer

Wait until your surgeon says so, usually a few weeks. The bigger problem is not the makeup itself, it is the rubbing and tugging involved in putting it on and taking it off.

The Academy's core recovery instruction after a corneal transplant is to use your drops exactly as prescribed, not to press on or rub the eye, and to wear glasses or a shield to protect the eye. Eye makeup runs against two of those. Applying liner or mascara means touching near the lid margin, and removing it usually means rubbing.

Makeup can also carry bacteria, and an eye with fresh stitches or a fresh incision is more vulnerable to infection. Old mascara and shared products are the worst offenders.

There is no published corneal transplant specific date for makeup, so ask at a follow up visit. When you do get the go ahead, start with fresh products, keep them off the lid margin and lashes at first, and take them off gently without pressing on the eye. Face lotions and hair products should be kept out of the eye too.

Call your surgeon now if you notice any of these

R - Redness: the eye turns red, or gets redder after it had settled down
S - Sensitivity: light sensitivity beyond what is typical for you
V - Vision: decreased vision, or a cloudy or foggy sensation
P - Pain: pain, irritation, or a foreign body sensation without a direct cause
Any of the RSVP signs at any time, including years later, since graft rejection can often be reversed if it is caught early
Signs of infection: increasing pain, thick or colored discharge, or a rapidly worsening red eye
Any blow, poke, or injury to the eye, because the wound edge stays weaker for life after a full thickness graft
Sudden severe eye pain or a sharp drop in vision in the first days after DMEK or DSAEK, which can mean the bubble has raised your eye pressure
A stitch that suddenly feels sharp, exposed, or broken

These warning signs after corneal transplant can point to problems that need same-day care. Do not wait for your next scheduled visit. If you cannot reach your surgeon's office, use their on-call line, or go to an emergency room.
